Person-centred care is ensuring that the care and support you give to an individual is tailored to their specific needs. Every individual is different and although they might have the same medical condition the same care and support they require is not always the same as one another, an example of this would be Dementia. You will need to ensure you have a clear understanding about the individuals you are working with. This will inclue their needs, the way they communicate with other individuals, their likes and dislikes, this is so you can promote and provide person centred care and support.
